Available services may vary depending on the patient's status, medical record, coverage, or connected systems.
032. Proper Use of the Account
The account must be used only by its owner or a person legally authorized to use it. Users must not: • Attempt to access another patient's account or medical record. • Provide false or misleading information. • Attempt to bypass security measures. • Disrupt the application or connected systems. • Use the service unlawfully or in a way that harms others.
043. Account and Security
Users are responsible for protecting their device and access to their account. Users should notify us if they suspect unauthorized use. We may take necessary measures to protect an account or temporarily restrict access when there is a security risk or suspected misuse.
054. Medical Information
The application and patient portal provide access to certain information associated with the patient's medical record. These digital services do not replace direct medical assessment when needed and should not be relied upon as the sole source of assistance in an emergency. In a medical emergency, users should immediately seek emergency assistance from the appropriate emergency services.

Appointments are subject to the availability of physicians and services. An appointment may be changed or cancelled, and the patient may be contacted when an operational change occurs. Users should verify appointment details and arrive on time.

Medical, financial, or regulatory records that must be retained under applicable requirements may continue to be retained after access to the application and patient portal is deleted.
