This page is for requesting deletion of your Balghsoon Polyclinic patient app and patient portal access account. You do not need the app installed to submit a request.
What happens after I submit?
Your request will be recorded and reviewed. We may contact you to verify your identity before completion. Requests are usually processed within a maximum of 30 days, and we will confirm once deletion is complete. Once completed, the access account is removed, while medical or financial records that must be retained by law are kept.
